A case of neurocardiogenic syncope that demonstrated the usefulness of repeated head-up tilt testing

Abstract
A 25-year-old man with recurrent syncope was referred to the outpatient clinic of arrhythmia service in order to evaluate the cause of his recurrent syncope. He underwent head-up tilt testing (HUT) to confirm neurocardiogenic syncope. At 10 minutes of HUT, he suddenly lost his consciousness. Electrocardiographic monitoring showed sinus arrest. He regained his consciousness and sinus rhythm resumed. He underwent a 2nd HUT 2 weeks later after taking atenolol. He lost his consciousness again and a sinus arrest redeveloped. A 3rd HUT was performed on him while taking theophylline. During 30 minutes of HUT, sinus rhythm maintained. However, he briefly experienced dizziness and a sinus arrest of 6.4 seconds was noted during isoproterenol provocation of HUT. After the 3rd HUT, we prescribed theophylline for 1 month. He did not show up at outpatient clinic after that. At 1 year follow-up, he stated that he had not taken theophylline since then. He has not experienced further syncopal episodes after the 3rd HUT.
